I understand I have to attend the annual Scholarship and Awards Banquet on August 25, 2026. Registration information is forthcoming and will be sent to the email you provided above. You must be in attendance to receive your funds. If you need to have an excuse reviewed please email Holly Hauck by July 1 with the excuse. The Dean will review your request.(Required)

These funds will be distributed in equal payments for the fall and spring semesters. Funds will only be dispersed after this form has been submitted and any appropriate reason for excusal at the scholarship reception has been approved by the due date.(Required)
